Grafting of autogenous bone remains valid as a gold standard for three-dimensional reconstruction of atrophic alveolar ridges due to its bioactive potential. The modification of the classical autogenous mandibular full-thickness bone block augmentation by means of the so-called Splitted Bone Block Technique shows an accelerated transplant revascularization as well as an improvement of the transplant volume stability both clinically and histologically.
Above all the soft tissue management remains an important factor for natural esthetic outcome and healing of the grafted area. In the context of vertical augmentation, the flap design must be individually adapted to the anatomical situation from a functional and esthetic point of view.
